Multimedia Consent Form

By signing this release form, I authorise Anah Project to use my name, pictures (a photographic, motion picture, and electronic), and recordings of my voice for all purposes in any media, language, format, or market, including distribution across our internal and external communication channels and to third parties. This includes exhibition, distribution, promotion, advertising, sale, press conferences, meetings, hearings, educational conferences, brochures, and other print media. This permission extends to all languages, media, formats, and markets now known or hereafter devised. This permission shall continue forever unless I revoke the permission in writing. I hereby waive the right to receive any payment for signing this release and waive the right to inspect or approve finished materials.
